On June 9th, 2021 our company name SurveyMonkey (SVMK Inc.) changed to Momentive, and we introduced a brand-new corporate site at momentive.ai!

All your login and account details are the same, and the SurveyMonkey product is not changing.

Does this change how I reach out to support?

No, you’ll contact and work with the same support teams you do today. We’re ready to help you reach your goals with our products.

Is my Customer Success Manager changing?

No, you’ll continue to be supported by our dedicated team of Customer Success Managers. If you have specific questions you’d like to discuss, please feel free to contact your Customer Success Manager or Customer Success team.

Our email addresses haven’t been updated yet, and we’ll let you know before they do.

Will the contracting entity be changing?

Yes, effective July 1, 2021, the names of our contracting entities will be changing as follows:

  • SurveyMonkey Inc. will become Momentive Inc.
  • SurveyMonkey Europe UC will become Momentive Europe UC
  • SurveyMonkey Brasil Internet Eireli will become Momentive Brasil Internet Eireli
Will I need a new contract?

No, there are no immediate changes or required updates to your existing contract with SurveyMonkey.

If you’re an Enterprise customer, over the coming weeks, the Customer Success team will communicate any changes to our corporate entity name, vendor forms, and other relevant billing adjustments.

How do I get a new vendor form or W9?

Please note, only the name of our company will change. All other details (our registration number, VAT, address and bank information) will remain the same.

If needed, please update our vendor name from SurveyMonkey to Momentive Inc or Momentive Europe UC, as applicable, in your organization’s records starting July 1, 2021. If your Enterprise team requires a revised form, please contact our renewals team at renewals@surveymonkey.com

Do I need to resubmit my tax exemption form for approval?

No, you don’t need to resubmit your tax exemption form. You’ll make the change to the new company name when you submit a renewal certificate in the future.

Will the name of the invoice I receive change?

Yes, effective July 1st, 2021, the company name on the next invoice you receive will change to Momentive Inc. or Momentive Europe UC, as applicable. For new customers, this may be your first invoice, for existing customers this may be your upcoming renewal invoice. However, all other details will remain the same, including our address, bank details and product names on the invoice.
