Contact Information Question (Demographic)

Using the Contact Information question type, you can collect respondent demographic information through a single question.

It isn't possible to add the Contact information question to new surveys. However, this question remains available on existing surveys.

By default, the following fields are available, but you can customize the field labels to collect different pieces of information you might need:

  • Name
  • Company
  • Address
  • Address 2
  • City/Town
  • State/Province
  • ZIP/Postal Code
  • Country
  • Email
  • Phone

You can also choose to hide some of the available fields by deselecting the Visible checkbox to the right of each field under the Edit tab.

By default, the State/Province field will display as an open-ended text field. However, if you know that all of your respondents live in the United States, you can choose to display the State field as Dropdown of U.S. States. This will make it easier for respondents to select the appropriate answer, and eliminate the chance of typos.

There is no way to display the Country field as a dropdown menu, but you can add a Dropdown question type right after the Contact Information question. You can copy and paste a list of countries into a Dropdown question, or use a pre-written question from the Question Bank.

Before you send out your survey, preview your survey design to see what your survey will look like to respondents.

You can't validate the Contact Information question type to require respondents to answer in a specific format, length, or range. Respondents will not encounter validation error messages, but they may see an error message if they don't answer all of the required fields that you define under the Options tab.

When you view your results in the Analyze Results section, you can use the Tagging Responses and Word Cloud features to identify recurring words or tag responses for each textbox.

Charts are not generated for this open-ended question type.
