How to Create an Eligibility Form

This article will outline how to add an Eligibility form to your site to ensure that your applicants meet the requirements of your available program(s). 

If this is your first time setting up an Eligibility Form, or if you do not currently have a form attached to your eligibility task in the site, you will need to create a new form.

To create a new Eligibility Form:

  1. Go to Settings
  2. Click on Eligibility
  3. Click Create Form
  4. Click on the Eligibility Quiz button or More Options > Edit to access the Form Builder
  5. Setup your form with the questions you will ask applicants to ensure they are eligible for your programs. 
    The form will be given "Eligibility Quiz" as the default title. However, you can edit the form title within the form. This will also update the button label within the Eligibility page. 

To edit an existing eligibility form:

  1. Go to Settings
  2. Click on Eligibility
  3. Click on the button that displays your Form Title or More Options > Edit to access the Form Builder
  4. Edit your form as required, however, be aware of the implications involved with editing a live form. For more information in regards to this, you can refer to How to Safely Make Changes to a Live Form
