File Upload Question

The File Upload question allows a user to upload a piece of information directly within the form they are editing.

  1. Within your form, click on the Questions tab
  2. Under Advanced Questions, double-click or click and drag the File Upload question to add it to your form
  3. Optional: Add an Identifier to your question
  4. Enter the Question Title
  5. Optional: Add a Description
  6. Set the +Display
  7. Set the +Options
    i. Decide if the Question will be Required or Optional
Display SettingsDescription
Append to previous questionThis question type appends to the question before it to make it appear that they are connected within the form.
NOTE: Any Branching Logic, such as to hide or show the previous question, will also apply to this question. However, the requirement settings won't be. If it is meant to be hidden it will need to be included in the branching logic as well.
OptionsDescription
OptionalEnabling this will make the question Optional.
When a user enters the form, if the question is Optional they will be able to proceed and complete the form without providing a response to this question.
Include an Optional description fieldAllow your users to add a caption/description to support their upload.
Allow Multiple UploadsIf enabled users can upload more than one file to this question.
If selected you can set a minimum and maximum number of uploads per question.
Maximum File SizeSet a maximum size for the defaults of up to 1000MB (1GB)
Limit File Formats Limit the format of the files you wish to receive. If you choose to limit the files, any format not entered won't be selected. This list is unrestricted and you can enter any format you wish to receive. Some suggested file formats are:
.pdf, .doc, .docx, .xls, .xlsx, .ppt, .pptx, .jpg, .jpeg, .png, .mp3, .avi, .mp4
